Incident Reports

Howdy fearless Melcher staff!

I wanted to share a bit of information that was passed on by Mary Wagoner. If you have any questions feel free to ask!

Hi!
There may rise an occasion when a staff member, other than a manager, needs to report an incident. All incidents should be reported within 24 hours of their occurrence. .
Incident report:
Step 1: Go to the Intranet: https://hplnet.hpl.lib.tx.us/
Step 2: Click on Forms.
Step 3: Click on Facilities.
Step 4: The second form on the right side of the page is the Incident Report Form.
Step 5: Choose the Word version.
Step 6: Complete the form. The information requested includes the following: date; time of incident; branch name and location, and a brief heading about the incident ( EX: property damage. The body of the report should include a brief, succinct description of the incident; names of the perpetrators, witnesses, law enforcement officials, customers or branch staff; location in the building or on the property. The next major portion is for the action taken by staff, GSD or police. If HPD or HFD were contacted, please complete the appropriate box. Please complete every section. The box at the bottom of the page pertains to when the incident reporter is completing the form. This section is often overlooked.
- Please walk through how to find the incident distribution list with staff.
(To find the distribution list log in to your outlook email. Next select the "To" link next to the address box. Type in "HPL" then search for Southeast Incident Reports.  Add this recipient to your email and please CC: Daisy Gutierrez.  Thanks a million!)
- Please be sure the type of incident is on the subject line of the email. Example: Incident – stolen cell phone

    Date-location-staff initials-subject-name of person involved.
